Here's a 5-string Tele-style instrument attributed to Steve Carr. Steve trained with Jimmy D'Aquisto and was a well-regarded East Coast luthier. He is best known as the guitar tech for KISS who built and modified the band's guitars to light up, emit smoke, and goodness knows what else—but he also found time for more conventional builds, including this. (These? I'm not sure if all the photos are of the same instrument.) Steve died in 2006.
